SCAN Medical Group ID: Where to Find It and What It Does

When doctors or specialists ask for your "Group ID", they almost always require your assigned Medical Group / IPA Name and Code.

Exact Card Location

Your Medical Group (IPA) is printed on the front of your SCAN card, directly underneath your Primary Care Doctor's name. It consists of the organization name (e.g., Heritage Provider Network, Astrana Health, Prospect Medical) and a 3-to-6 digit numeric IPA or GRP code (e.g., 0418).

How Medical Groups Work in the SCAN HMO Network

SCAN Health Plan operates as a Medicare Advantage Health Maintenance Organization (HMO). In an HMO model, clinical oversight is decentralized:

  • Specialist Referrals: Your primary doctor coordinates with your Medical Group to approve visits to cardiologists, orthopedists, and other specialists.
  • Prior Authorizations: Diagnostic imaging (MRIs, CT scans) and outpatient procedures are authorized by your Medical Group's utilization management committee, not by SCAN corporate.
  • Claims Delegation: Providers bill the delegated IPA directly for professional services. Review our provider billing guide for EDI payer ID and verification workflows.

Common Medical Groups Contracted with SCAN

Depending on your geographic region in California, Arizona, Nevada, or Texas, your card may list medical networks such as:

• Heritage Provider Network / Regal / Lakeside
• Astrana Health (formerly ApolloMed / NMM)
• Prospect Medical Group
• Torrance Memorial Integrated Physicians
• St. Joseph Heritage Medical Group
• Optum Care Network / Monarch Healthcare
